To enable you to collect valuable feedback from your seminar participants, we've designed a Participant Evaluation form that you can customize to suit your needs. This form includes a section that participants can use to indicate that they're interested in scheduling a consultation with you. As a seminar leader, your feedback is important to us too, so please fill-out a Leader's Evaluation form and forward it to us, after you complete your first seminar.
